THIS IS A LOAN. IT IS SEPARATE FROM ANY DIRECT LOANS YOU MAY RECEIVE AND WILL NOT AUTOMATICALLY BE COMBINED WITH YOUR DIRECT LOANS UPON LEAVING SCHOOL. IN ORDER TO CONSOLIDATE THIS LOAN WITH DIRECT LOANS, YOU WILL HAVE TO INFORM YOUR CONSOLIDATING LENDER THAT YOU HAVE A PERKINS LOAN THAT YOU WOULD LIKE TO INCLUDE IN A CONSOLIDATION.


You are required to read this statement. By checking the box next to this statement you are agreeing to the terms and conditions laid out in the statement below. If you do not check the box next to this statement, you will not be allowed to submit this application.

The accepting of a loan is a serious legal obligation; therefore, the conditions and terms of the loan should be understood and agreed on in advance. In signing this document, you are agreeing that you understand the terms, conditions, and responsibilities of the loan and you agree to honor these obligations. This loan will be reported to a national credit bureau at least annually.

The Perkins loan has a 5% fixed interest rate. The loan can be prepaid at any time, without penalty. The maximum amount that can be borrowed for one academic year is $5,500 for undergraduates and $8,000 for graduate students. The maximum aggregate amount that can be borrowed is $27,500 for undergraduates and $60,000 for graduate students.

  1. I understand that I must, without exception, report any of the following to the Financial Aid Office immediately:
       a. If I withdraw from school
       b. If I transfer to another school
       c. If I change to below half-time status
       d. If my name or address should change

  2. I understand that I must complete an exit interview when I graduate or withdraw from the University. If I fail to do so, a hold will be placed on my transcripts.

  3. I understand the terms of the loan as outlined on the Promissory note.

  4. I understand that my loan funds may only be used for direct expenses of attending Stephen F. Austin State University and that I may be prosecuted if I use these funds for any other purpose.

  5. In the event that my Perkins Loan becomes delinquent, I authorize Stephen F. Austin to take the following action:
       a. Withhold readmission to the University
       b. Withhold all official transcripts and grades
       c. Withhold any degree to which I may otherwise be entitled
       d. Report all necessary information to the federal or state government or to other educational institutions.
       e. Assign the loan balance to an outside collection agency.

  6. I certify the Student Financial Aid Office at Stephen F. Austin State University has provided that opportunity to ask any and all questions pertaining to the loan.

  7. I understand that my Perkins Loan can be consolidated. Consolidation is refinancing all of your federal student loans into one loan through a private lender. This new loan will have a new interest rate, payment terms, and monthly payments. Some of the benefits available in the original student loan may be lost after consolidation. To apply for a consolidation loan, contact your Stafford Loan lender and be sure to mention that you have received a Perkins Loan from Stephen F. Austin State University.
